I watched a directors cut of The Dark Knight, most of the acting in this scene wasn't acting. Most of the cast hadn't seen Heath Ledger in his make up as The Joker. So, the loud tray drop was legit, the shocked looks, legit and the actress (i forget her name) that plays Rachel was legitly uncomfortable looking at Heath, which is why he kept saying Look at me.
