The psychiatrist doesn't refer to him by name in this scene, in the way the previous one did. This says to me that, having reached the end of this story, we don't know if Arthur was ever his real name. As Joker emerges, the man in front of us has become a mystery, his identity unknown, his history open to interpretation.

Take note of the grey hairs and the darker look in his eye. We're seeing post-Batman Joker realising he inadvertently created his own archenemy. The second victim looks like he was in killing joke. This make joker is best villains of all comic book even he not has super power but he broke 2 greatest superhero(batman,superman) and got last laugh.
